Patient Transport Service – Booking

 

Only health care professionals, staff in hospitals, aged care facilities, private hospitals, nursing homes and general practitioner's (GP) surgeries can make patient transport bookings. Patients and carers may make a patient transport booking only if they have been provided a medically authorised transport form by the General Practitioner.

Important information for health professionals to provide when making a PTS booking

When making a PTS booking, you will need to provide information about the patient and their medical conditions. Please ensure you have the following information before making a booking:
  • the name, age, date of birth and gender of the patient
  • the patient's home address
  • the pick-up location for the patient, this may be a residential address, medical clinic, or hospital (include the ward or department)
  • the date and time required for pick up
  • the location of the medical facility (including ward or department) the patient is going to
  • the name of the medical officer (doctor) authorising the transport
  • the nature of the patient's illness or injury
  • any special care requirements
  • any depressed immunity or infectious disease information
  • any return transport requirements.
